When you visit the Site, and make a specific request (such as a request for information or a request for an appointment) certain information is required to complete that request. If a request is made for service that can only be completed through the US mail, you will be asked to provide a mailing address. The information that is collected is used only to complete the request and is not saved or used for any other purpose without your consent.
Collection of Implicit Information
