Medical Records Requests
First Care Clinic outsources our release of information process to HealthMark Group.
HealthMark fulfills all patient requests for personal copies at no charge to the patient. Patients can request medical records by going to https://requestmanager.healthmark-group.com. Patients will need to enter an email address and register for an account.
Once logged in, patient can select Submit Request from the menu options and enter all required fields to submit an authorization to HealthMark directly. They will process medical records requests and provide notification via mail or email once complete. Records will then be made available for patients to download through HealthMark’s Request Manager.

Patient Forms and Information
First Care Clinic utilizes electronic forms upon check-in. Please arrive to your appointment 15 minutes early to complete the check-in process. Insured patients need to bring their insurance card along with their co-payment to their appointment. Patients who wish to apply for the sliding fee scale need to bring their proof of income and sliding fee payment to their appointment.
